Forms

If not ordering electronically, complete, print, and send a Microbiology Test Request Form (T244) with the specimen (http://www.mayomedicallaboratories.com/it-mmfiles/microbiology_test_request_form.pdf).

Necessary Information

1. Organism identification and specimen source are required for processing.

2. Each slant must be submitted under a separate order.

3. An email address registered on the Mayo Medical Laboratories website is required for report delivery.

 

Shipping Instructions

1. See Infectious Specimen Shipping Guidelines in Special Instructions for shipping information.

2. Place specimen in a large infectious container (T146) and label as an etiologic agent/infectious substance.

3. Place all isolates submitted for whole genome sequencing (WGS) together in one large bag, and send it in the same shipping container. This is necessary for comparison of isolates by this method.

Advisory Information

Currently only Staphylococcus aureus is being tested by this method. For other bacteria, order PFGE / Bacterial Typing by Pulsed-Field Gel Electrophoresis (PFGE). PFGE orders for S aureus will be automatically converted to BTWGS / Bacterial Strain Typing by Whole Genome Sequencing.
